Position Summary

Essential Responsibilities
- Respond to weekend on-call requests and provide skilled nursing care to hospice patients.
- Perform patient assessments and monitor changes in condition.
- Administer medications and treatments as prescribed by the physician.
- Educate patients and families on symptom management and hospice philosophy.
- Communicate with the interdisciplinary team to ensure continuity of care.
- Document all visits and interventions accurately and promptly in EMR.
- Provide crisis intervention and emotional support to patients and families.
Qualifications
- Current Texas LVN license.
- Minimum of 1 year of nursing experience (Hospice preferred).
- Reliable transportation and valid auto insurance.
- Strong communication and interpersonal skills.
- Proficiency in EMR systems (experience with Consolo preferred).
