What is GUIDE?

The GUIDE model (Guiding an Improved Dementia Experience ) is a program from the Centers of Medicare & Medicaid Services which offers exclusive benefits for caregivers and patients living with dementia who have traditional Medicare, at no cost to the patient, beginning July 1, 2025.

Goals of GUIDE

Improve quality of life for people living with dementia

Reduce burden and strain on unpaid caregivers of people living with dementia

Prevent or delay long-term nursing home care

Key Features of GUIDE

Comprehensive Care Coordination

Access to a GUIDE navigator to develop a care plan, schedule monthly home visits, and coordinate local resources

Respite Care

Recognizing the demands placed on caregivers, eligible patients and families can benefit from up to $2,500 in annual reimbursement for in-home, adult day center, and facility-based respite care.

Cost Coverage

All services under GUIDE are offered at no cost to Medicare beneficiaries

Caregiver Support

Access to educational materials and a 24/7 support line

Who is Eligible for the GUIDE Program?

A person is eligible if they:
☑ Are enrolled in Medicare Parts A & B and it’s their primary payer
☑ Reside in the designated geographic area
☑ Have an eligible dementia diagnosis
☑ Are NOT enrolled in a Medicare Advantage plan
☑ Are NOT enrolled in a Program for All-Inclusive Care for the Elderly (PACE)
☑ Are NOT enrolled in hospice
☑ Are NOT residing in a long-term skilled nursing home
