Who is Eligible for the GUIDE Program?
A person is eligible if they:
☑ Are enrolled in Medicare Parts A & B and it’s their primary payer
☑ Reside in Denton, Collin, Dallas, or Tarrant County
☑ Have an eligible dementia diagnosis
☑ Are NOT enrolled in a Medicare Advantage plan
☑ Are NOT enrolled in a Program for All-Inclusive Care for the Elderly (PACE)
☑ Are NOT enrolled in hospice
☑ Are NOT residing in a long-term skilled nursing home, assisted living facility, or memory care facility
