Are you a caregiver feeling overwhelmed? We understand the unique challenges of dementia care. Get ready for a new level of support! Our innovative GUIDE program is set to begin on July 1st, offering comprehensive assistance designed specifically for dementia caregivers.

The GUIDE model (Guiding an Improved Dementia Experience ) is a program from the Centers of Medicare & Medicaid Services which offers exclusive benefits for caregivers and patients living with dementia who have traditional Medicare, at no cost to the patient. GUIDE participants will have access to a GUIDE Navigator to develop a care plan, schedule monthly home visits, and coordinate local resources.

Our Program Director and Care Navigator, Brittany Jenkins MSW, has been working diligently to create a program that truly makes a difference in the lives of dementia caregivers. Brittany will have a direct impact on caregivers by offering one-on-one support, connecting to resources, navigating the healthcare system.

Brittany is a compassionate and experienced social worker specializing in gerontology. She holds a Bachelor of Social Work from West Texas A&M University and a Master of Social Work from Grand Canyon University. Brittany has provided essential support to older adults and their families across skilled nursing facilities, hospice care, and palliative care settings. With a deep understanding of the unique physical, emotional, and social needs of aging populations, she is committed to delivering person-centered care that honors dignity, promotes comfort, and enhances quality of life.
